If a player takes his helmet off on his own during the play, are the officials supposed to blow the play dead immediately??

Only if it's the runner, the player with the ball.
Would it be a penalty if a player intentionally takes his helmet off during the play?
No. Not sure why a player would intentionally remove his helmet during a play. If a player other than the runner loses his helmet during a play and continues to participate, it's a penalty. He would be endangering himself.
